  Sports Physicals & vaccinations need to be scheduled, call (608) 489-8280. Everything else is available on a walk-in basis.
  
  Billable to insurance or $30 cash pay option. No other discounts apply. Athletes under age 18 must be accompanied by a parent or have a signed parental consent. Not intended to replace your annual wellness exam.
